Tramadol is a synthetic opioid analgesic, used for pain management after fractures or soft tissue injuries. It is also intended for the treatment of cancer (in patients with stage-three or stage-four cancer) and post-surgical pain.
Tramadol can be prescribed to the patients over 14 years of age. The medicine comes as ampoules, rectal suppositories, and tablets. Tramadol quickly causes mental and physical dependence. Therefore, it is registered as a controlled drug.
Effect on the body
At therapeutic doses, Tramadol has no effect on the respiratory center, blood pressure and consciousness. There have been reports that the short-term use of Tramadol slows down intestinal motility without causing constipation.
Tramadol acts on brain and spinal cord. The drug works by activating sodium and potassium channels, stimulating opioid receptors, depressing the respiratory center and slowing the heart rate.
Tramadol starts to work within 15-20 minutes (however, injections take effect within 1-3 minutes). The drug binds to plasma proteins and is metabolized in the liver. Tramadol is partially excreted unchanged by the kidney.
Tramadol acts by stimulating the opioid receptors. The drug affects central and peripheral nervous system. Tramadol contributes to the active transport of sodium and potassium ions in the spinal cord, thereby reducing the transmission of pain impulses by the nerve fibers.
Tramadol inhibits the reuptake of norepinephrine in synapses, due to which this substance is evenly distributed throughout the body. This reduces pain sensitivity and calms the patient down.
Side effects of Tramadol
Tramadol can cause side effects even at therapeutic doses. At high doses, the drug provokes the following physical symptoms:
- Gastrointestinal tract: sickness, dry mouth, abdominal pain, bowel disorders, flatulence.
- Cardiovascular system: increased heartbeat (tachycardia), sudden drops in blood pressure, collapse, fainting.
- Kidneys: difficulty urinating, urinary retention (oliguria).
- Sex organs: erectile dysfunction, menstrual irregularities.
- Immune system: allergic reactions of any severity (from urticaria to anaphylactic shock).
- CNS: sweating, vertigo, unsteady gait, muscle spasms, seizures and loss of coordination.
- Other: shortness of breath caused by respiratory depression, headache, severe agitation.
Side effects of Tramadol are dose-dependent: the larger the dose, the higher the risk of adverse reactions.
Adverse effects of Tramadol
Since Tramadol is highly addictive, it may provoke behavioral changes in drug addicts. These reactions differ from patient to patient, but the most common of them are:
- mood swings, memory impairment;
- sleep disturbance, apathy.
Other possible side effects include red eyes, constricted pupils, skin color changes, rash, unsteady gain and excessive hand gestures.
The long-term use of Tramadol can cause destruction of bone tissue; in patients treated with Tramadol, bones become weak and break easily. The use of this drug for two years or more can lead to the development of such diseases as liver cirrhosis, hepatitis, epilepsy, dementia and other mental disorders.
Overdosage with Tramadol can cause sickness, fainting and epileptic seizures, as well as vision impairment, drops in heart rate and shortness of breath. In such situation, the patient must undergo detoxification that usually lasts several weeks.
